Step 8 - Submit NMLS Form (MU4)
Submitting the Individual MU4 is the step that technically creates your mortgage originator license application. As part of this process you will also authorize a criminal background check and fingerprinting at the federal level, a credit report and provide any needed information for the legal disclosure questions.
Have the following on hand before you start the MU4:

• Residential history, including to and from dates (mm/yy) and addresses for the past 10 years.
• Employment history in any financial related services, including to and from dates (mm/yy) and addresses for the past 10 years.
• All names used since age the age of 18, including alias, aka, maiden name, etc.
• You will be required to pay the applicable fees (licensing, criminal background check, credit report and NMLS processing fees); available payment methods are Visa, MasterCard, or ACH.
